People with frailty have significantly higher healthcare use, experiencing 2-3 times more use compared to their non-frail counterparts. Furthermore, frailty leads to increased morbidity, mortality, and reduced quality of life. Frailty may also result in functional decline, higher fall risk, prolonged hospital stays, and premature entry into residential aged care facilities.
Thankfully, many causes of frailty can be effectively managed, and in some instances, reversed to improve health outcomes and quality of life. Identifying older individuals who may be living with frailty is therefore of paramount importance.
We recognize the significance of frailty management and healthy aging, which is why we have developed a range of resources to support your efforts in this area. These include a comprehensive Frailty and Healthy Ageing QI Toolkit, as well as a QI Action Plan example, both of which are designed to help you meet PIP QI and accreditation requirements.
