This population health management and clinical audit tool has been developed and designed by GCPHN in partnership with GPs and practices over the last five years.
This tool is currently being used by 110 practices on the Gold Coast and is being rolled out by 10 other PHNs nationally, which could see the tool used in up to half the general practices around the country.
Primary Sense is fully subsidised by GCPHN. It is simple to use and creates real time data reports without manual data extraction.
It supports proactive and opportunistic approaches to improve the health outcomes for your patients with patient list reports and prompts for missing items of care.
With the added bonus of risk stratification, identifying those patients most at risk, that may have slipped through the gaps during COVID-19 is as simple as three clicks and a report is generated.
