Changes to referral templates for mental health

Gold Coast Primary Health Network (GCPHN) has recently updated referral templates for all PHN-funded Mental Health Programs and the Persistent Pain Program.

The referral templates that have been changed are:

  • Mental Health Stepped Care Services referral. The programs included on this referral are:
    • Low Intensity Groups, NewAccess
    • Psychological Services Program
    • Severe and Complex Care Coordination – Plus Social Program
  • Psychological Services Exceptional Circumstances referral
  • GCPHN Persistent Pain Program referral

Changes to the Stepped Care Services and Psychological Services Exceptional Circumstances referral templates:

  • a healthcare card number is now required on the form

Changes to Persistent Pain referral template:

For Word and clinical software templates:

  • Medical Objects address for secure messaging added
  • Email address removed
  • Fax number (reiterate the new fax number)

For clinical software templates only:

  • Automated fields for patient clinical history added
