A digital health pathway is now available to support the development of the child or young person’s health management plan that can be shared with child safety, health professionals and other key people in their life. The management plan outlines the child and young person’s health needs, professional responsibilities and timeframes.
This system helps ensure that children and young people in care have their health needs identified and met. By working together and sharing information, knowledge and expertise, we can make a real difference to the lives of children and young people in out-of-home care.
