HITH would like to trial a new model whereby GPs can refer suitable and eligible patients directly to the service to avoid a hospital admission and instead have care delivered at home.
The service will commence for patients with urinary tract infections or cellulitis where GP treatment options have been exhausted but patient acuity is such that medical management in the home is feasible. Once identified and accepted to the service clinical governance will rest with the health service.
In addition, Gold Coast Health is set to trial accepting direct referrals for its newly established Multidisciplinary Avoidance and Post-Acute Service (MAPS) from invited GP practices.
The MAPS team is comprised of nursing and allied health professionals and has been established to provide a hospital avoidance pathway for GPs and the Queensland Ambulance Service (QAS). MAPS delivers a rapid in home clinical response seven days a week and have the capacity to visit the referred person at home within 24 hours from referral if required. The service provides assessment and practical bridging intervention for up to two weeks for individuals on the program after which will need to establish transfer of care processes to community care providers that need ongoing support. Medical governance of patients will remain with the GP.
To trial these services HITH are inviting local practices to express interest in being involved in a pilot of GP Referral to HITH and MAPS.
