MyMedicare is a new voluntary patient registration model for general practice.
The program will strengthen and formalise the relationship between patients and their primary care teams, drive improvements in the delivery of continuity and quality of care, improve patient health outcomes, offer additional/expanded access to some telehealth consultations, and provide a platform to support funding reform towards blended payment models.

Tracey Johnson, CEO Inala Primary Care. Tracey is the CEO of Inala Primary Care, a charitable, teaching and research-active practice in Queensland’s most disadvantaged suburban location. The practice delivers over 50,000 consultations a year through a large multi-disciplinary team. Through partnerships and creative funding, the practice is on the road to building three community health precincts over the next three years. In 2015 she undertook a Winston Churchill Memorial Trust Fellowship studying how to bring care out of hospitals and into the community. She remains research active through co-designing new models of care at the practice and studying health outcomes. She is a member of the Evaluation Working Group of the Australian Healthcare Homes Program and deputy chair of the Primary Care Advisory Group of the Australian Institute of Health and Welfare. To support practices and build performance, she co-founded Cubiko, a practice management dashboard software solution, in 2018. She continues to advocate for policy change and funding reform in multicultural health, patient self-management support, integrated care and chronic disease management. She is working on a national framework for practice managers and helped lead the MyMedicare process into practices across Australia.
