A webinar for practice teams facilitated by AGPAL in conjunction with GCPHN.

Team-based care uses multidisciplinary teams (which may involve new staff, or the shifting of tasks among existing staff). This webinar explores with speaker Susan Raphael, the application of team-based care in practice, and provides practical strategies for the embedding of these changes.

Speaker

Susan Raphael, Organisational Management Consultant. Susan is a qualified Social Worker and a catalyst for innovation. She helps individuals, teams, organisations and communities with sustainable strategies related to all aspects of people and relationships in workspaces. Her expertise includes leading and implementing accreditation assessments, quality improvement, planning and review processes within the not-for-profit, government and private sectors. Susan holds a Master’s degree specialising in organisational development and an International Diploma in Narrative Practice.
