GCPHN has commissioned care finders to support improved integration between the health, aged care, and other systems at the local level within the context of the care finder program.  This is part of the Connecting Senior Australians to Aged Care as part of the Royal Commission.

The Gold Coast’s new care finder program, officially launching on 27 April, is part of a national initiative funded by the Department of Health and Aged Care (DoHAC) in response to a recommendation of the Royal Commission into Aged Care Quality and Safety. 

GCPHN has commissioned three providers to deliver, the care finder program on the Gold Coast. This program aims to provide specialist and intensive assistance to help people in the target population to understand, and access aged care and connect with other relevant supports in the community. Care finders will complement existing aged care support services, providing assistance to people who require intensive support to interact with My Aged Care. The aim of the care finder program is to improve outcomes of the care finder target population by improving understanding of aged care services, how to access and stay connected to services once commenced. 

You are cordially invited to attend the official care finder program launch. This is an opportunity to meet the providers, GCPHN representatives, and other relevant stakeholders. Registration is essential for catering purposes.
