You’ve diagnosed your patient with dementia – what next?
Dementia Australia is funded by the Australian Government to be a key linkage from diagnosis to support. With an expanded suite of post-diagnostic supports and programs, Dementia Australia exists to ensure that every person, pre, during and post-diagnosis is supported to enable access to disease management information, resources, supports and services.
We understand that it isn’t always possible to maintain regular contact with your patient after giving them a diagnosis and this is where Dementia Australia comes in. We are here to proactively contact people who have been newly diagnosed to offer information and support about their diagnosis and access to an extensive range of post diagnostic support programs. We will agree services with each client to ensure they have access to what they need at the time they need it – and that referrers and GPs are kept informed.
During this one-hour webinar, you will hear from a panel of experts on Dementia Australia’s role in the dementia pathway, how we can support you and what you can do as a health professional to ensure your patient is most effectively supported after a diagnosis of dementia.
