Chronic obstructive pulmonary disease (COPD) remains an underdiagnosed condition and a significant contributor to morbidity, especially within primary care settings. During the 2019-2020 period, COPD was the sixth leading cause of potentially preventable hospitalisations in the Gold Coast region, with a rate of 223 per 100,000.
Gold Coast Primary Health Network (GCPHN) is seeking expressions of interest (EOI) from general practices with a particular interest in COPD, to collaborate with and develop and test a new COPD Quality Improvement (QI) activity aiming to improve the clinical management of patients with a diagnosis of COPD and increase use of exacerbation plans.
The purpose of general practice involvement in the co design of a topic specific quality improvement activity is to provide general practice perspective and advice to the GCPHN project team.
Over the next five months, the general practice will work with GCPHN QI team to develop and test a COPD QI activity resulting in QI resources and planning for education event/s that are designed to meet general practice local care needs and expectations.

The general practice must be available to begin the co-design from Monday 14th October 2024, with engagement activities including practice visits, online MS teams meetings, phone and email correspondence and event attendance to support completion of activities.
The general practice will be remunerated for the GP, PM and PN time involved in the co-design activities that occur, with opportunity for GPs to participate in a RACGP approved clinical audit.
