The project aims to foster relationships between aged care, general practice and the Supportive and Specialist Palliative Care Service, and assist RACFs in meeting the requirements for the Aged Care Quality Standards.
RACF’s participating in the SPACE Project will participate in monthly ‘Palliative Care Needs Rounds’. Palliative Care Needs Rounds are triage meetings and will help identify and prioritise RACF residents at risk of needing palliative care interventions and symptom management.
