The most common cause of cold chain breaches are power outage, fridge malfunction, human error or high usage. It is essential to have an up-to-date Vaccine Management Protocol (VMP) in place based on the National Vaccine Storage Guidelines – Strive for 5 (3rd edition) to guide you in the event of a cold chain breach at your practice.
If we don’t protect our vaccines, they won’t protect our community
In the event of a cold chain breach (below +2°C or above +8°C – not including temperatures up to +12°C for less than 15 minutes), isolate vaccines and report to QHIP as soon as possible on QHIP-ADMIN@health.qld.gov.au. QHIP will then contact the GCPHU to follow up with your practice. Do not discard any vaccines unless instructed by GCPHU.
