Gold Coast Health is working to return gynaecology patients to the care of their GPs faster, with improved communications and a rapid re-entry pathway if required.
GPs will receive a discharge summary at the time a procedure is undertaken but can expect to receive an updated one when the results become available.
The second document will include the pathology report, a follow up plan and advice for the GPs.
In addition, should GPs have any concerns with the report or advice provided, or the patient’s condition changes, a dedicated Clinical Nurse Consultant (CNC) will be available 8am-4pm, Monday to Friday to take calls. The CNC can provide advice or link GPs to consultants and registrars.
Details for the rapid re-entry process will be included in the discharge summary.
