Over the past year GCPHN and Gold Coast Health have been working in partnership to develop a Joint Regional Plan for Mental Health, Suicide Prevention, Alcohol and Other Drugs Services.
As part of this joint planning process, discussions with a range of stakeholders emphasised the need to support system navigation for both service providers and community members; the Gold Coast service system is constantly evolving and it can be confusing to navigate, resulting in people not getting the care they need.
In response, GCPHN has developed a printable resource to summarise the range of services commissioned by GCPHN.
This resource is part of a range of recent activities that have considered how GCPHN can better support system navigation by adopting a more consistent approach in how we communicate about the services we fund. This new resource aligns with the updated Mental Health Stepped Care Referral Form and redesigned GCPHN website.
Printed copies have been sent to all General Practices in the Gold Coast region. To request more copies of this resource please contact communications@gcphn.com.au
