National Advance Care Planning Week challenges everyone to discuss what living well means to them and to consider who they would want to speak for them if they could no longer communicate or make their own health care decisions.
GCPHN has developed The Advance Care Planning QI Toolkit which is a practical guide for general practice to implement Advance Care Planning (ACP) as a CQI activity and for PIP QI and CPD purposes. The toolkit aims to increase the number of ACP documents completed with patients.
To complement this, GCPHN has also developed the ‘Planning your future care today’ online and printed booklet to encourage all adults to start the ACP conversation and complete ACP documents.
