New and updated referral templates on HealthyGC

New templates available on HealthyGC

Referral templates for Sexual Health and Oncology are now available on the GCPHN website.

Updates to existing templates

Please be advised that the following changes have occurred to referral templates recently:

  • Persistent Pain referral has been updated with consultants name  (updated 27/05/2019)
  • Persistent Pain indefinite option within the duration of referral section has been removed and now only provides a 12 month option (see section 5). (updated 16/04/2019)
  • Urology UTI referral is no longer available and has been removed from the HealthyGC website (updated 16/04/2019).
  • Urology Other has been amended to a generic Urology template and includes UTI fields (see section 6). (updated 16/04/2019)
