Cardiovascular disease (CVD) is a serious issue in Australia. Early identification of risk and the offer of support to improve individual modifiable risk factors can reduce the incidence of preventable CVD events.
The My health for life program targets adults at high-risk of developing CVD and other chronic conditions and works with them to make lifestyle choices to improve their overall health. To help you identify patients who may be eligible for referral to the My health for life program, Gold Coast Primary Health Network have created a continuous quality improvement (CQI) activity example ‘High Cardiovascular Risk and My health for life’ for CAT4 users, which you can use as part of your Practice Incentive Program (PIP) Quality Improvement (QI) requirements to undertake CQI.
The template can be found on our PIP QI webpage under the Quality Improvement tab. For any questions or support relating to CQI, contact our Practice Support Helpdesk on 5612 5408 or email practicesupport@gcphn.com.au. My health for life is also offering practice visits to interested practices.
