To ensure continuity of care for residents, Wound Specialist Services, will offer a private fee-for-service to Gold Coast RACHs. The following changes will apply:
Further referral pathways for wound care will be communicated to Gold Coast RACHs. The Wound Pilot in Residential Aged Care Homes (RACHs) project is a nurse led in-reach program conducted by Wound Specialist Services.
The Wound Pilot project has been open to all 57 RACHs across the GCPHN region and aims to improve outcomes for people with chronic and complex wounds who are living in RACHs through clinical services and an education program to build capability of RACH staff in the provision of wound care.
For participating RACHs the Wound Pilot has been very successful in providing many positive outcomes for both staff involved in consultations and learning opportunities but also in reduced healing times and improved quality of life for residents living with chronic and complex wounds.
