Supporting Minds is a mental health service that provides 3-month, moderate intensity structured psychological supports and other structured interventions such as group-based lifestyle interventions. This service has replaced the Psychological Services Program.

What is the eligibility criteria for supporting minds?
Supporting Minds has two service streams with differing eligibility criteria.
Note: It is desirable (not essential) for people to have a Health Care Card
How can my patients access Supporting Minds?
You can refer your patients directly to Supporting Minds by sending a referral form to the Primary and Community Care (PCCS) Head to Health Phone Service.
When you send a referral, the PCCS H2H Phone Service will keep you informed of the service that your patient has been referred to.

Alternatively, you can ask your patients to call the Head to Health phone line directly where they will receive an assessment from one of their trained mental health clinicians to determine the most appropriate referral option to their meet treatment goals.

What services are available for people not eligible for Supporting Minds?
If you have a patient who is not eligible for the Supporting Minds program, you can refer them to Head to Health for an assessment and direction towards a program appropriate for them.
