This is a dedicated wound clinic with a wound specialist GP (Dr Stephen Yelland) and his team of highly trained nurses, that support local general practitioners and their patients to improve health outcomes with people that have chronic, complex, recurrent, and debilitating wounds that seriously impact on their quality of life. This clinic is supported by GCPHN, and the team at the clinic liaise closely with all providers involved in the patient’s care.
The Chronic Wound Clinic offers assessment and review, access to advanced therapies such as Ultrasound Wound Debridement and specialises in compression therapy.
The clinic liaises with the referring GP to develop a wound management plan, whilst maintaining the referring GP as the centre of the patient’s care. The multi-disciplinary approach to managing wounds may often require the clinic to liaise with the referring GP re referrals to Specialists or allied health
The ultimate goal is to refer back to the usual GP for ongoing management once a plan is in place. The GP will receive a detailed assessment and treatment management plan after review. When required, the Chronic Wound Clinic will support care until the point in time when the wound is improving and can be discharged back to the usual GP.
If significant vascular issues are known, please refer in the first instance to a Vascular surgeon at Gold Coast Health or private surgeon depending on patient circumstances.
Prior to referring, please review the referral template for recommended tests and information to be completed or added to expedite the triaging process and access to appointments.
Once the referral has been assessed, the Wound Clinic will contact the patient directly to book an appointment and answer any other questions the patient may have.
