Gold Coast general practices will receive their GP Practice Grants Program Financial Acquittal Form templates soon.
While the due date of the acquittal form is not until 31 July 2024, we would encourage you to complete and return the form as soon as your GP Grant Program funds have been expended.
For full details of reporting requirements please refer to item F within your Grant Agreement (page 6). The evaluation template will be sent to you in due course.
As a timely reminder please refer to Item 10 in the General Grant Conditions (page 7 of your Grant Agreement) for details of record keeping and compliance requirements. Provision of these records will be required should you be subjected to an audit/compliance check.
