My health for life is a free, Queensland Government funded integrated risk assessment and behaviour change program that supports Queenslanders in decreasing their risk of chronic diseases, specifically diabetes, heart disease and stroke.
The program is funded through Health and Wellbeing Queensland and delivered by the Healthier Queensland Alliance, led by Diabetes Queensland and made up of the National Stroke Foundation, Heart Foundation, Ethnic Communities Council of Queensland (ECCQ), Queensland Aboriginal and Islander Health Council (QAIHC) and all seven Queensland Primary Health Networks (PHNs).
