The RACH Wound Pilot project is moving to a fee-for-service model. Charges will apply from 01 November for all new case appointments.
The Wound Pilot in Residential Aged Care Homes (RACHs) project is a nurse led in-reach program conducted by Wound Specialist Services. The Wound Pilot project has been open to all 57 RACHs across the GCPHN region and aimed to improve outcomes for people with chronic and complex wounds who are living in RACHs through clinical services and an education program to build the capability of RACH staff in the provision of wound care.
To ensure continuity of care for residents, Wound Specialist Services, will offer a private fee-for-service to Gold Coast RACHs. The following changes will apply:
From 01 November charges will apply for all new case appointments and
From 01 December charges will apply for all review appointments.
A Specialised Wound Care Assessment pathway has been developed on Gold Coast Community HealthPathways to support General Practitioners navigating appropriate referral pathways. For more information and to review the full summary of the pilot including outcomes click here.
