This free webinar will provide an outline of the HIV Nursing Model of Care used at Alfred Health, Victoria. This will include the use of electronic medical record data to assess and drive improvements in HIV quality of care, innovative nurse led approaches to provide quality care to people living with HIV who struggle to engage within a standard public hospital outpatient framework, and reduce the risk of these patients becoming “lost to follow up”.
