Gold Coast Primary Health Network (GCPHN) undertakes a range of activities to identify priorities in our region. Through these activities including our annual needs assessment and engagement with our local community, healthcare professionals and other relevant stakeholders and groups we identify health care priorities for the region.
We plan solutions in partnership with stakeholders and identify opportunities to engage local service providers. We will call for tenders then appoint and commission the most suitable providers to meet the needs of our region.
We also seek Expressions of Interest (EOIs) for a number of our activities, which are advertised in ‘Current opportunities’ below.
Gold Coast Primary Health Network (GCPHN) is seeking grant applications from primary health care providers who regularly service Gold Coast Residential Aged Care Facilities (RACFs) and are interested in enhancing their video telehealth equipment and capabilities to enable RACF residents’ greater access to primary health care. Please note, the grant funding excludes pharmacists and existing GCPHN Commissioned services.
The Primary Care Telehealth Support Grants are an opportunity for Primary Care Providers to purchase the required equipment or enhance their current information technology infrastructure, which will support increasing the number of primary care video telehealth consultations offered to RACF residents.
Up to $2,000 funding is available per primary health care provider and can be spent on overcoming any existing barriers to offering video telehealth consultation with RACF residents. This could include purchasing equipment or resources such as:

The Royal Commission into Aged Care Quality and Safety (Royal Commission) examined the complex issues faced by Senior Australians at the interface of the primary health care and aged care systems. These issues included a lack of access to general practitioners (GPs) and allied health professionals in RACFs, difficulties accessing after-hours care and inappropriate transfers to hospitals, which contribute to poor outcomes for Senior Australians and increased pressure on the health system.
The aim of the project is to ensure there is timely access to primary health care professionals in RACFs through enhancing access to video telehealth. This will be achieved through ensuring both primary care providers and RACFs have adequate virtual consultation facilities and technology that enables residents to access clinically appropriate telehealth care with their primary health care professionals. Please note, all Gold Coast RACFs have previously been offered the opportunity to apply for grant funding.
To be eligible the applicant must be a primary health care provider who provides frequent services to Gold Coast RACFs or plans to, with an average of 5 or more health care consultations per week. This is based on the average number of consultations by GPs in RACFs per week (Royal Commission into Age Care Quality and Safety, 2020 pg.6-7). This is a grant for unique primary health care providers including General Practitioners and Allied Health providers. Pharmacists and current GCPHN Commissioned service providers will be excluded. Responses will be evaluated against the grant conditions and if applicants are not deemed eligible or further information is required, the project team will communicate this via email.
