Aged care is a top priority for Gold Coast Primary Health Network (GCPHN), which funds service providers to carry out various activities, including:

1. Early intervention services for chronic disease management.
2. Support for Residential Aged Care Homes (RACHs) to facilitate telehealth access to primary healthcare providers.
3. Promotion of after-hours care plans in RACHs.
4. Creation of pathways to enhance access and awareness of dementia services.
5. Deployment of Care Finders to help older Australians navigate aged care services and community supports.

The overarching objective is to enable people to remain at home longer, delaying entry into RACHs and reducing unnecessary hospitalisations for older Australians.
