The Statewide Office of Advance Care Planning presents an annual virtual education forum for health professionals in Queensland to share the latest developments and research supporting quality advance care planning.

This event is suitable for health professionals located in Queensland working across hospital, community, aged care and research settings.

Explore a patient’s journey through the advance care planning lens at the 2025 advance care planning virtual forum.

Presented over four days by the Statewide Office of Advance Care Planning (ACP), you will be guided through each phase of the ACP process and take away practical strategies you can apply in your professional practice.

The 2025 virtual forum will take place from Monday, 17 March to Thursday, 20 March, featuring virtual lunchbox sessions from 12:00pm to 1:00pm AEST.

We encourage you to make the most of the virtual forum as a team-based learning opportunity. Gather your colleagues for lunch, ask questions, and engage in conversations about how advance care planning can be applied in your workplace.

• All healthcare professionals have an important role to help with planning and ensuring people have choice in their care.
• Ideally, ACP will result in a person’s preferences being documented to help ensure these preferences are respected.
• ACP conversations should be routine and occur as part of a person’s ongoing healthcare plan.
