In collaboration with Gold Coast Primary Health Network (GCPHN) and Gold Coast Health RACH Acute Care Support Services (RaSS), the Enhanced After Hours Project team invites GPs and nurses working in residential aged care homes to an engaging forum addressing the complex care needs of residents living with dementia. Esteemed speakers will share their expertise, experiences, and practical resources to support health professionals in providing high-quality dementia care.

Participants will gain insights on topics including:

• End-of-life care
• Identifying and addressing unmet needs
• Navigating medication complexities

Attendees will also be introduced to the latest tools and support networks available to enhance care delivery.

End the day with an interactive Q&A session, designed to foster collaboration and address challenges through solution-focused discussions aimed at improving care for residents living with dementia.

Learning Objectives

• Gain a deeper understanding of the nature of changed behaviours in residents living with dementia and the expression of a behaviour as an unmet need.
• Improve knowledge and understanding of end-of-life care for residents living with dementia.
• Evaluate medication management for residents living with dementia.
• Gain awareness of available resources and apps to assist in caring for residents with dementia.
• Develop greater awareness of the wider health system challenges associated with caring for residents with dementia.
• Reflect on personal and organisational challenges and experiences surrounding caring for residents living with dementia. Participate in a room-wide Q & A panel session to develop actionable solutions to these challenges, supporting a shift to a proactive culture and approach.
