Strengthening First Nations Health Together

Gold Coast Hospital and Health Service, in partnership with Gold Coast Primary Health Network invites general practitioners, primary health care nurses and primary health care teams to explore key Gold Coast Health First Nations programs that are delivering meaningful improvements in cardiac care, chronic disease management and smooth navigation across the healthcare system. The session blends clinical clarity with cultural awareness, offering practical knowledge ready to use the next day in your practice. 

The webinar will explore the following available key First Nations Services and more:  

• Better Cardiac Care – This segment covers the coordinated approach supporting First Nations patients at higher risk of cardiac disease. Participants will learn how early intervention, proactive follow-up and culturally grounded communication create safer and more effective heart-health journeys. 

• Mungulli Chronic Disease Program – The Mungulli program is designed for continuity, connection and community-centred chronic disease care.  

• A&TSI Nurse Navigation Service – This service provides structured, patient-centred navigation throughout the health system. Nurse Navigators support patients in transitioning between primary care, hospital services and community programs, reducing fragmentation and improving follow-through.
