QI Infographics
After the evaluation stage is complete for facilitated QI activities, GCPHN create a 1-page infographic. This is a great way to showcase the fantastic work undertaken by practices (supported by GCPHN), in a simple and visually pleasing way. We house these on our website, to celebrate the successes.
GCPHN’s Healthy Ageing and Frailty QI Infographic highlights a successful initiative addressing frailty in seniors supported by Bollen Health. Frailty affects 25% of individuals aged 65+, identified by fatigue, weakness, mobility challenges, major illness, and weight loss. When an individual has even one sign of frailty, they will lose their ability to bounce back from illness or an accident. They are at greater risk of falling and are more likely to end up in hospital or at a residential aged care home.
The overarching aim of the QI activity was to provide education and coaching to general practice teams on how to best support older patients through improved completion of health assessment in patients aged 75+, better identification and management of frailty (using the ‘’FRAIL Scale’’), and promotion of other elements of care that support healthy ageing.
