Cardiovascular disease (CVD) is a general term for conditions that affect the heart and blood vessels, including heart attack and stroke. It is common in Australia, but many cases can be prevented by managing risk factors early and knowing when to get help.
Cardiovascular disease is one of the leading causes of illness and death in Queensland and across Australia, but much of the risk is linked to factors that can be improved. Looking after your heart and blood vessels can reduce your chance of serious illness and support your overall health and wellbeing.
A Heart Health Check is a 20-minute check up with your GP and nurse to assess and help you understand your risk of having a heart attack or stroke in the next 5 years. Importantly, it can also inform what to do to prevent and lower these risks.
You are eligible for a heart health check if you do not already have heart disease and are 30 years and over.

Risk increases with a combination of factors. Common risk factors include older age, smoking, high blood pressure, high cholesterol, diabetes, being above a healthy weight, physical inactivity, unhealthy diet, kidney disease, and a family history of heart disease or stroke. Often, culturally and linguistically diverse communities, women, people living in rural and remote areas, and people experiencing social or economic disadvantage are at risk of poorer health outcomes. We call these communities priority populations. Some people may not realise they are at increased risk, so regular check-ups are important.

My Health for Life is a free chronic disease prevention initiative. Participants work with qualified allied health coaches through 6 or more coaching sessions delivered over 14–18 weeks. The initiative supports healthier habits and achievable lifestyle changes.

The COACH Program is an evidence‑based chronic disease prevention and self‑management program. COACH offers free telephone based counselling for people with chronic conditions by providing structured one‑on‑one health coaching via phone or telehealth.

A 12‑week intensive active lifestyle program delivered by Bond University Allied Health Professionals bi-weekly. It is for patients in the community at risk of frailty or early admission to aged care facilities. Includes group health education classes, exercise classes and individual sessions with allied health professionals. More information.

A free, evidence‑based digital support program for patients diagnosed with coronary heart disease. Support includes information and advice for patients over a 12 week period in addition to educational resources, access to the MyHeart MyLife Facebook Community and an online dashboard.
