My Health Record (MHR) is a safe and secure place to keep your health information, available to you and your health care providers anytime, including in an emergency.
You are always in control of your record. You can update your settings at any time and decide what information you would like to store.
My Health Record contains important health information that you and your healthcare provider organisations have added, all in one place.
Healthcare providers like doctors, pathologists, and pharmacists can upload clinical documents to your record. Your record can include certain Medicare information held by Services Australia, depending on the settings that you choose. You can add the health information that you think is important or may want to have available in an emergency. Your COVID-19 vaccination dashboard in My Health Record brings together information about your COVID-19 vaccinations, booster, test results uploaded and related health information.
What can be uploaded?
To set up your My Health Record, you first need to link My Health Record to myGov. More information can be found here.
You can view and update your personal information, add emergency contacts, set up email and SMS notifications, check you Medicare information or cancel your record. More information can be found here.
You can manage your child’s health information, including vaccination records, allergy information and medicines they are taking. If your child is under the age of 14 years and on the same Medicare card as you, you can also create a My Health Record for them if they don’t already have one. More than one parent can manage a child’s record as their authorised representative. For more details, please see how to access or create a child’s record.
To take control of your My Health Record, you will need to prove your identity. My Health Record uses details from Medicare to verify your identity. You may need to know information such as:
If you can’t answer the identity questions or would prefer to speak to someone in person, you can call the My Health Record help line on 1800 723 471.

When you take control, the information and documents that were in the record when it was managed by your authorised representative will still be in the record. Healthcare providers may have also uploaded clinical documents and viewed information in your record.
If you don’t take control of your record, it will continue to exist. Healthcare providers will still be able to view information in the record and upload clinical documents to it.
My Health Record has information in languages other than English to help people from non-English speaking backgrounds learn more about My Health Record.

On 26 November 2018, the Australian Parliament passed legislation to strengthen privacy protections in My Health Records Act 2012 without debate or division. For those who decide they no longer want one, it can be cancelled at any time. The information in your record, including any backups, will be permanently deleted from the system.
