The Royal Commission into Aged Care Quality and Safety (Royal Commission) specifically recommended better access to information and advice on dementia, and support services available in the community.
GCPHN has worked closely with Dementia Australia and a broad range of stakeholders to develop a dementia consumer pathway resource for those living with dementia, their families, and carers. This resource includes a repository of local services and supports, questions to consider asking your doctor, and helpful check lists for the first 6-months after receiving a dementia diagnosis.
Download a print friendly version of the Dementia Consumer booklets here and here.
This short survey is designed to assess the effectiveness of the GCPHN Dementia booklet ‘Dementia: Resources and support to help you live well‘. Your feedback is valued as it will help to improve future consumer resources. |
