IMPRINT (Integrated Multidisciplinary Proactive Risk Identification and Navigation Team) is a new community based multidisciplinary team which aims to reduce potential hospital length of stay through early navigation and intervention of Gold Coast Health residents who have recently been approved a high-level Home Care Package (HCP) by the Aged Care Assessment Team (ACAT) before they are admitted to hospital.
