My health for life is an evidence-based health initiative for people at risk of developing chronic disease. The initiative supports eligible Queenslanders to live and age well and minimise preventable health risks. It is funded by the State government through Health and Wellbeing Queensland.
My health for life expanded its eligibility criteria in 2021 to support more Queenslanders to make healthy lifestyle changes and reduce the risk of chronic disease.

How to refer a patient
My health for life is a free, six-month program available to all eligible Queenslanders through telephone health coaching, small group, or video sessions. It is also offered in-language in partnership with the Ethnic Community Council of Queensland. The Queensland Aboriginal Islander Health Council supports the delivery of the Aboriginal and Torres Strait Islander offering.

Nick is an accredited Exercise Physiologist and Exercise Scientist with a passion for helping others move, improving their lifestyle and modifying their health holistically.
Nick has had experience working with a range or different conditions and clients in both NSW and QLD. Nick is passionate about helping improve the quality of life of individuals, making movement enjoyable and sustainable healthy behavior change. Nick works with a variety of different clients but specialises in healthy behavior change, diabetes/metabolic disease, cardiopulmonary rehabilitation and working with individuals with disabilities. Nick runs a range of different group programs at Griffith Health Clinics, whilst also providing individualised, tailored exercise programs and physical activity interventions. |

Jarrod is an Accredited Practicing Dietitian that aspires to provide high quality and friendly Dietetic service to his community.
He considers himself approachable and is passionate about helping people become the best version of themselves. Jarrod believes in taking a holistic approach when discussing health and nutrition. He not only analyses your diet, but he also considers all the aspects of your life: social situation, emotional factors, environmental factors, attitude, values, and beliefs around food. This allows him to tailor recommendations that will reinforce sustainable healthy eating habits that reflect your personal goals. |

Kelly graduated from the University of the Sunshine Coast with a Bachelor of Clinical Exercise Science in 2016. She started her career in the private sector and has experience in DVA, NDIS, Medicare EPC, Private, HSS and Telehealth services.
More recently Kelly has moved from the Sunshine Coast, where she was working in two clinics, treating a variety of cardiac, metabolic, musculoskeletal and mental health conditions. She is also interested in pilates and hydrotherapy as an alternative to gym-based exercises. In her spare time, Kelly enjoys surfing, hiking, camping and traveling. Qualifications and memberships:

Amber is an Accredited Practising Dietitian who graduated from Griffith University on the Gold Coast.
Amber has a passion for creating healthy habits that are sustainable, fulfilling and optimise positive health outcomes. She has experience working in multiple settings including hospitals, private practice, and aged care. Her varied case load includes diabetes, metabolic syndrome, weight management, gastrointestinal disorders, eating disorders, thyroid issues, and paediatrics. Amber emphasises on the fact that there is “one size fits all” approach and ensures to adapt her advice to your individual needs, goals, social determinants, lifestyle, values, and beliefs. |

Participants are also able to complete the program through telephone coaching if there are no local sessions available in your area.
