Gold Coast Primary Health Network commissions a range of primary care and aged care funded services to meet the diverse and growing needs of the Gold Coast region. Our commissioned services are available at no cost to patients.
Details of our specific commissioned services for primary care and aged care are outlined below, however here are quick links to other information which may be useful to health professionals working in this space.
CareFinder – a free service for older, vulnerable Gold Coast who need intensive assistance to access aged care services at home. It is specifically for people who have no-one else who can assist them and are eligible for government-funded aged care.
Complex Wound Clinic – only accessible with a GP referral, this service provides wound management in communication with the patient’s usual GP and specialist services.
Endometriosis and Pelvic Pain Clinic – a multi-disciplinary service to improve the healthcare outcomes for people that experience the debilitating effects of Endometriosis and Pelvic pain that seriously impact on their quality of life.
Frailty and Healthy Ageing Programs – we have a choice of three services to increase the quality of life for older people in our community and help live longer at home. The fully-funded programs are available for people over the age of 65 (or 55 if identifying as an Aboriginal or Torres Strait Islander person) with the referral of a GP or Allied Health professional.
Medicare Urgent Care Clinic – when a patient is unable to see their usual GP and requires urgent (non-emergency) medical care. An emergency department alternative where a discharge summary is provided to the patient’s regular GP.
The Care Finder program is a free service for Gold Coast residents. It helps older, vulnerable people, who need intensive assistance to access aged care services at home.
