Gold Coast Primary Health Network Partnerships

Gold Coast Primary Health Network (GCPHN) works closely with public, private and non-government organisations in the primary health care sector and across the acute care sector to improve the health and well-being of the Gold Coast. GCPHN is building ‘one world class health system’ for the Gold Coast and we know we can’t do it alone. To achieve this goal, we work closely with diverse groups, sharing knowledge and skills, working to improve the efficiency and effectiveness of our primary care system, ensuring it meets local needs.

Integrated Care Alliance

Gold Coast Health (GCH), in conjunction with the Gold Coast Primary Health Network (GCPHN), have formed the Integrated Care Alliance (ICA) and are working to create an integrated, coordinated, seamless and streamlined world class health system on the Gold Coast that strengthens the links between primary, secondary and tertiary care services to ensure a truly person-centre approach to service delivery.

Gold Coast Primary Health Network (GCPHN) works closely with Gold Coast Health in a range of programs to improve coordination of care to ensure patients receive the right care at the right place at the right time by the right person.

General Practice Gold Coast (GPGC) is an independent membership organisation that promotes and supports general practitioners and their practices.

The Gold Coast Primary Care Partnership Council (PCPC) first came together in 2006 as a voluntary collaboration between organisations from government and non-government sectors. The PCPC represents a broad range of primary care providers on the Gold Coast. Bringing together diverse organisations on the Gold Coast, the PCPC provides an opportunity to share information and improve coordination for the achievement of positive health outcomes for the Gold Coast Community.
