A statewide issue has been identified regarding the quality of clinical information provided on radiology request forms, including:
illegible handwriting (for non-ieMR sites)
reason for order / details’ field in the ieMR left blank
no referrer contact details or provider number provided.
It is acknowledged that radiology requests and reporting is a complex issue and a statewide project to improve radiology reporting is underway.
In the interim, without including clinical information, it is very difficult for radiology staff to perform an accurate scan or test which can result in inappropriate tests, duplication of tests or unnecessary delays to care. It particularly disadvantages patients who have travelled from a regional or remote site specifically for the test.
All clinicians are reminded to provide a full and proper radiology request by completing all relevant fields. If this detail is not provided, the test may not be performed and the request may be returned.
