Gold Coast Primary Health Network currently funds three Healthy Ageing programs, available at no cost for eligible participants.
These programs challenge the notion that frailty is an inevitable part of ageing and are designed to empower individuals to enhance their overall quality of life and live longer independently. Each program provides unique offerings, so you can find the best option that suits your needs.
If you are 65 years or older (or over 55 for First Nations peoples), all you need is a referral from your doctor or an allied health professional saying you show just one sign of frailty. If you are a pensioner or on a health care card, the program is free.
When you have even one sign of frailty, you lose your ability to bounce back from illness or an accident. You’re at greater risk of falling and you’re more likely to end up in hospital or at a residential aged care home.
