Sample workflow for general practice utilising Primary Sense report Patients with High Complexity (5 and 4):

1. Report Download: Practice Manager downloads the Primary Sense report ‘Patients with High Complexity (5 and 4)’* monthly to identify complex patients with potential overdue/eligible items of care (Medication review, GPMP, GPMP review, TCA, TCA review).
2. Team Assignment: Practice Manager shares the Primary Sense report with the receptionist or nurse to schedule appointments for these patients with missing items of care.
3. Patient Follow-Up: The receptionist or nurse contacts patients in report via phone or text message (supported by Primary Sense) to arrange visits for the potential overdue/eligible items of care**.
4. Patient Visit: Patients attend the practice and receive the necessary care items identified via the Primary Sense report.
