Reducing hospitalisations in heart failure and CKD – preventing readmissions after heart failure discharge and avoiding hospitalisation in CKD.
Join an expert GCUH faculty for a practical masterclass focused on two key priorities: improving post-discharge Guideline Directed Medical Therapy (GDMT) alignment in heart failure to reduce rehospitalisation, and strengthening community-based optimisation of GDMT in CKD to help slow progression and improve outcomes.
The session will combine practical guidance on overcoming barriers to therapy optimisation and exploring local pathways and processes for enhancing continuity of care between hospital and general practice.
