GCPHN EOI for funded Healthy Ageing and Frailty Prevention Project

Gold Coast Primary Health Network (GCPHN) is offering 15 practices the opportunity to participate in a remunerated project focusing on Healthy Ageing and Frailty Prevention.

The aim of this project is to improve outcomes for community dwelling older persons and increase screening for frailty at the general practice level.

This remunerated project will run from May 2024 – December 2024.

Why frailty?

People with frailty utilise healthcare two to three times more than their non-frail counterparts and experience higher morbidity, mortality as well as lower quality of life. Frailty can increase functional decline, increase the risk of falls, contribute to longer length of hospital stay and contribute to early entry into Residential Aged Care Homes.

Many of the causes of frailty can be managed and, in some cases, reversed to create better health outcomes and quality of life. It is therefore important to identify older people who are living with frailty.

  • Payment for participation: total $7,500 excluding GST (3 payments of $2,500 throughout the project).
  • GPs can accrue 9.5CPD hours through participation in an accredited RACGP clinical audit “Improving Care of the Older Person through a Health Assessment’’.
  • Improve Quality of Life: Help our community’s older population live longer and better at home. Implement routine frailty screening for those aged 65 and above, aiding early identification and interventions.
  • Receive practice-based support to implement a QI activity, including access to a toolkit and action plan template.
