Gold Coast Primary Health Network (GCPHN), Gold Coast Health, Queensland Ambulance Service and Kalwun are working together to develop a Joint Regional Older Persons Strategy (JROPS) – a shared plan to improve health and wellbeing outcomes for older people in the Gold Coast region.
The JROPS will outline a clear, collaborative roadmap for better service integration across primary care, aged care, hospital services and community support, ensuring older people receive coordinated, equitable and person-centred care.
To help shape this work, Impact Co. has been engaged to lead stakeholder engagement and strategic planning. Local general practices, nurses and primary care teams are invited to participate in one of three Stakeholder Workshops to share insights, challenges and opportunities for improving care and coordination for older people.
