2. Risk stratification: Practice Manager sorts the report by the ‘ACG score’ column to ensure patients with the highest complexity (score of 5) are followed up first*.
3. Team Assignment: Practice Manager shares the report with the receptionist or nurse to schedule appointments for these patients for missing items of care.
4. Patient Follow-Up: The receptionist or nurse contacts patients in report via phone or text message (supported by Primary Sense) to arrange visits for health assessment**.
5.  Patient Visit: Patients attend the practice and receive the necessary care items identified via the Primary Sense report.
