The Head to Health Assessment and Referral Phone Service provides multiple entry pathways (website, phone, and face-to-face) into mental health services that best meet the consumers’ needs through their preferred platform, GPs, psychiatrists, and other health professionals may also use the H2H phone service to find service options for their patients.
The objective is to offer a seamless care pathway for consumers to receive the right level of care at the right time to meet their mental health needs.
PCCS are a well-established local mental health service provider who had previously been commissioned by GCPHN to manage the Gold Coast Intake, Referral, and Triage program, established in 2018 in the Gold Coast region and operated until 30 June 2022 at which time it transitioned over to the Head to Health Assessment and Referral Phone Service.
The H2H phone service has established data/phoneline service processes, an extensive regional provider database, and a clinical team trained with and using the Intake, Assessment, and Referral Decision Making Tool (IAR-DST).
