APNA’s Cultiv-8 Nurse Clinics program, delivered in partnership with Gold Coast Primary Health Network (GCPHN), is an 8-week hybrid learning program with weekly themes designed to support the introduction of the nurse clinic concept.
Utilising APNA’s eight Nurse Clinic Building Blocks as the foundation to guide nurse clinic implementation, program participants will be provided with the tools required to plan a nurse-led, team-based model of care which meets the health needs of their community. These clinics often focus on preventative care (such as immunisation), education and lifestyle support to help patients better manage their conditions and improve their overall health outcomes.

The Cultiv-8 Nurse Clinics Program is not only a professional development opportunity, but also a strategic initiative that aligns with Australia’s national health reform agenda. It equips primary care teams with practical tools to deliver sustainable, evidence-based care in line with emerging policy priorities.
Cultiv-8 empowers nurses to take a more proactive role in chronic disease care through structured assessment, care planning, education, and follow-up. These models enhance access and continuity, while helping general practices respond to Chronic Conditions Management (CCM) changes from 1 July 2025, that promote team-based care and greater involvement of non-GP clinicians.

Cultiv-8 helps unlock the full potential of the primary health care nursing workforce, enabling nurses to work to their full scope of practice in a supported, structured way.

Each Cultiv-8 Nurse Clinics program will support participating general practices across an 8-week hybrid learning program. General practices may assign up to two representatives to participate in the program. Participants must include at least one nurse and may include one supporting staff member, GP or another nurse from each participating general practice.
