Consider how you can incorporate The Way Back Support Service (TWBSS) Out of Hospital pathway into your care planning for patients experiencing suicidality – early referral can make a meaningful difference - shares Dr Jardin Taha.
Earlier this year I met a woman in her fifties facing depression, fleeting suicidal thoughts, and a storm of social pressures. She had unstable housing, a painful separation, long term unemployment, and a return to heavy drinking as she tried to cope. She wanted help, had hope for recovery, and was committed to regular review and follow up.
I organised a mental health care plan over the next few visits, referred her to a psychologist, and linked her with drug and alcohol services. But I was reminded that recovery often hinges on more than medical care alone. The social, environmental, and economic challenges tied to suicidality may not always be addressed by therapy or medication in isolation.
That’s where The Way Back Support Service (TWBSS) Out of Hospital (OOH) pathway is helpful for general practice. This free and accessible service connects patients (aged 16+) experiencing suicidality with a Support Facilitator within one business day.
Over a period of up to three months, the facilitator works with the patient to develop a personalised safety and support plan, while linking them to community and health supports including housing, financial advice and social connection.
The model has already demonstrated positive outcomes for individuals following hospital admission for suicidal crisis through its partnership with Gold Coast Health. The expansion to an Out of Hospital pathway allows GPs to refer at-risk individuals earlier, providing preventative support before escalation to crisis or ED presentation.
Integrating a TWBSS OOH referral alongside a Mental Health Care Plan supports a more holistic, coordinated response without affecting access or eligibility to Medicare subsidised psychology visits. By providing clinical care with social prescribing, we can give patients a stronger safety net and a better chance at lasting recovery.
