GCPHN collects personal information for the purpose of assisting its activities and functions in the primary health care sector. If your contact details are collected they may be used to forward information and notifications from GCPHN. In some circumstances we may provide information to our funding agency (Department of Health and Aged Care) or to service providers that enter into legal contracts with us which are bound by confidentiality.
There is no legal requirement for you to provide your personal information, however if you choose not to disclose your personal information this may exclude you from our services and programs. We do not routinely disclose information overseas.
If you wish to access and/or correct errors in your personal information held by us, or you suspect there has been a breach in privacy, this should be put in writing via email to feedback@gcphn.com.au, or by letter addressed to: Privacy Officer, Gold Coast Primary Health Network, PO Box 3576, Robina Town Centre QLD 4230. We will notify you of the outcome in less than 30 days, and if you are not satisfied with our response, we will inform you of the process to make a complaint.
