The GCPHN has developed the ‘Planning your future care today online information and PDF Booklet to encourage all adults to start the ACP conversation and complete ACP documents. It is supported by the PDF booklet and has simple step by step information about ACP in Queensland. It outlines the processes all adults (regardless their age or health status) need to follow to ensure their documents are in order if they are unable to speak for themselves. This resource can be shared with patients or any adult enquiring about ACP.

The Australian Government department of Health has released the 2021 National framework for advance care planning documents. The Framework outlines ethical considerations and best practice principles relating to ACP nationally.  The Framework can be used by anyone, including health professionals and consumers. However, its primary audience are policy makers and administrators of ACP programs in the health and aged care sectors.
