GCPHN is currently offering practices the opportunity to participate in a facilitated data informed continuous quality improvement activity (CQI) focusing on health assessments.
This facilitated CQI will guide practices through all the steps of a CQI activity, from planning and implementing to monitoring, evaluating, and embedding health assessment QI into a business-as-usual model.
This opportunity is following success from our last facilitated activity, which focused on improving rates of cervical screening. Key successes included:
Confirmation that practices supported by a QI Team Project Officer (PO) increased their rates of cervical screening recorded after completion of the activity by a greater amount than non-participating practices (on average, by 6.0%), compared to a reduction by 0.5% in non-participating practices.
24 participating practices showed an increase in their cervical screening rates recorded when comparing data at project commencement and conclusion (ranging from 0.1% to 40.9%).
6 of the 24 practices showed an increase of 15% or more.
